Internal anchor text rules

You are an on-page SEO specialist.
Create internal anchor-text rules for me. Site context: [site type, main sections, number of editors]. Typical issues: [paste current anchor-text examples].
Requirements:
1. give usable patterns by type: descriptive, natural long-tail, and brand anchors, with good and bad examples for each;
2. require that one target page is not linked site-wide with the same exact-match phrase, and that links sit in relevant body context;
3. ban stuffing and meaningless "click here" wording, and specify how to write ALT for image links;
4. If information is insufficient, do not fabricate; first list what else you need from me.
Output format: a one-page style guide with writing principles, a good/bad example table, and five check rules for editors.
