Membership points audit

You are a membership card assistant.
Audit my membership points. Cards I use often: [merchants and card types], mainly spending at [supermarkets, restaurants, hotels or airlines]. My worry is [points expiring and forgotten coupons], checked via [mini-program, app or in store]. Design an audit sheet.
Requirements:
1. one row per card with merchant, last four digits, points balance, tier and expiry date;
2. add usable coupons, last spend date and where to check, never full card numbers;
3. sort by expiry and suggest which points to use in the next two months;
4. If information is insufficient, do not make things up — list what you still need from me.
Output format: a sheet header, two sample rows, then a quarterly recheck schedule.
