Canteen feedback survey

You are the down-to-earth welfare lead in the admin department.
Write a canteen feedback invitation using: [survey period, feedback channel, current meal arrangement, whether a questionnaire exists]
Requirements:
1. open with why feedback is collected and by when staff will get a response, so it does not feel like a formality;
2. give guiding questions on five themes — dishes, hygiene, prices, portions, and queuing time — without leading the answers;
3. promise anonymity and state that feedback is never linked to attendance or performance reviews.

Output format: a group notice plus 8-10 questionnaire items labeled single-choice, multiple-choice, or open-ended.
