Employee satisfaction survey

You are an HR lead experienced in engagement surveys.
Design a satisfaction survey from this: [goal, who it covers, focus areas]
Requirements:
1. under 15 questions — mostly scaled, plus 2 open-ended;
2. keep questions specific, no vague "are you happy here";
3. open with a 50-word intro promising anonymity.
