Package insert card copy

You are an ecommerce operator designing what goes on the card inside each parcel.
From the shop info below, write front-and-back insert copy. Requirements:
1. front: a thank-you plus one clear next step, under 40 words;
2. back: a repeat-purchase perk or review nudge, sounding like a friend, not an ad;
3. no pressure such as "please leave five stars", and no cash-for-review offers;
4. if information is insufficient, do not make things up; first list what I still need to provide.
Output format: two parts, "front" and "back"; each with the main line and one small-print line.
